About the Opportunity
Gilgandra Shire Council has a rewarding opportunity for a dedicated Registered Nurse Clinical Care Coordinator - Home Care Package & Jack Towney Hostel to join their Aged Care & Disabilities team.
This position is offered on a full-time, Monday - Sunday basis with participation in an on-call roster for after business hours.
About the role
Provide leadership and support to the Service with a strong emphasis on accountability in the delivery of clinical care. This includes promoting and guiding advanced, holistic, and evidence-based practices in line with contemporary standards, and ensuring appropriate clinical supervision of staff in collaboration with the Service Manager.
Day-to-day duties may include (but will not be limited to):
- Consulting with residents/representatives, allied health professionals and other relevant stakeholders;
- Managing care plans, including wound care and outcomes within the Facility;
- Supervising staff clinical practice, in line with Best Practice and Policies and Procedures
- Demonstrating effective and positive communication with all stakeholders through established channels and lines of communication;
- Promoting a teamwork spirit within the care team and between all service types;
- Ensuring all documentation is in compliance as per policy, procedures, ACFI and Accreditation; and
- Following WH&S guidelines, including infection control and reporting mechanisms for general welfare and safety of self, all staff, residents and others on the premises.

About You
To be considered for this position you will be a Registered Nurse with current registration with AHPRA and demonstrated Aged Care experience.
Gilgandra Shire Council is looking for an individual with strong interpersonal skills and the ability to communicate effectively to other staff and departments.
Finally, you will be competent in computer literacy, specifically Microsoft Office Suite. Experience and knowledge of ACFI will be held in high regard.
Please Note: A current, unencumbered drivers licence is essential.
